Print the order form and fill it out carefully.

Send your order form and your clearest original picture (up to a maximum of 8" x 10") to:

will return your original photgraph to you. Please put your
name and address on the back of the photo.

All products will be received within 30 working days. Please add $12.00 shipping and handling for each order.
